The survey is expected to take between 30 and 60 minutes. Please use data from Calendar Year 2025 (CY 2025) for your responses. The information submitted will be used to produce a report that highlights the collective impact that Friends groups provide to their site and the wider OPRHP system. As part of the survey collection process, OPRHP has developed a survey toolkit. Upon completion of the survey, you and your OPHRP facility contact will receive a copy of your submission.
